Madoka is everywhere at all times, but in the rare instances she manifests on a plane perceivable by mortals, she takes the form she last held before ascending to godhood. That is of a fourteen year old girl in an extremely frilly pink outfit. She has pink hair held in two somewhat spiky pontyails by a larg darker pink bow. Around her neck is a dark pink bow tied in the back.
Her outfit is very "lolita" inspired and is a pin in the rear-end to fully describe. See her Picture for full details. It has three shades of pink and white.
She has also shown in the past the ability to manifest in a plane white dress that hangs down as far as her usual outfit does, or simply as a nude being of "energy".

Abilities/Powers
Madoka possesses two key abilities. The first is that she is omnipresent and ominisicient (Please see Other about this). She is everywhere at all times (past, present, and future) and knows all things.
Her second ability relates to the magical girls that create contracts with Incubators. When they use up all their magical power, she appears to them and saves them from their wishes causing harms to others. She lets them be at peace, and has shown a willingness to grant a final desire so long as it does not interfere with the actual world (they can visit a possible future, for example, in the form of a spirit).
Skills
She is extremely kind and holds potential for endless love and hope. She was never very good in school when she was still human, but those are abilities of the past, of a world that never was and never will be.
Personality
Madoka dislikes fighting and suffering even more. She believes in the dreams and hopes of all sentient creatures and thinks it is deplorable that anyone should ever have to suffer for their impossible wishes. She is kind and loving, but her knowledge of all also means she is aware that she can not interfere much if ever in the affairs of the mortal worlds. Her hope is her greatest aspect, for no matter how long (eternally) she must fight, she will always hold it and her virtues dear.
History
Madoka's history begins in one timeline of one world, where the world's most powerful witch, a "Walpurgis Night" manifested over her home. She was granted a wish to become a Puella Magi by a mysterious creature that called itself Kyubey. Her wish was to save a cat that would have otherwise died. In exchange for her wish she was to spend the rest of her life fighting witches. A friend she had recently made in school, a sickly girl named Homura Akemi, ended up getting trapped in the dangerous pocket reality made my witches known as Barriers, and she and fellow Puella Magi Mami Tomoe defended Homura from an attack.
From that point on, Homura would accompany Madoka, Mami, and eventually others such as Sayaka and Kyoko as they fought witches - although Homura was not a Puella Magi herself, she desperately wanted to be of use to her good friend Madoka. When Walpurgis Night appeared, it would kill all of Madoka and Homura's friends. Madoka would choose to sacrifice her life to fight the witch, much to Homura's horror.
In another timeline, Homura ran up to a surprised Madoka in class and told her that she had become a Puella Magi, and they could fight together! This surprised Madoka who was greatly embarassed by the action of the new girl, but they ended up working together. Madoka would die again. This would repeat itself in various manifestations, bundling Homura's sorrow more and more tightly around Madoka. Another timeline would have Homura and Madoka die together with them aware that a Puella Magi who uses up her power becomes a witch. Madoka used an artifact known as a Grief Seed to save Homura, asking her to use her time powers to go back and prevent her from ever becoming a Puella Magi, no matter what.
And via this request, Homura became cold and seemingly cruel, doing anything and everything within her power to prevent Madoka's descent into the role of Puella Magi. Madoka could not get her, but she couldn't help but feel bad when Homura collapsed into her arms and told her that Madoka could never understand how she was her closest friend ever, but she would give anything for Madoka to not become a Puella Magi!
Walpurgis Night descended as she had in many timelines before that. Madoka knew she had to do something for Homura, and ran to her aid. Homura was getting her tail handed to her by the powerful Walpurgis Night, and Madoka said that she would solve this. She made her wish to Kyubey, but it was quite different than any wish ever before, and it could be granted as a Puella Magi's power level was dictated by the despair surrounding them... and Homura had put countless timelines around one center point - Madoka.
Madoka wished for the power to stop every witch before she was born. Due to the very building blocks of the universe, this turned her into a god and rebuilt the entire universe in a magically fuelled big bang. She became a god so that no Puella Magi would ever suffer for their wish ever again - and the universes forgot about her, as she had never existed.
...all but Homura.
Other
Did I just do what it looks like? Did I just sign up a literal GOD? Yes, yes I did. But take heart! While she exists she will have very little interaction with other characters. Canon has shown she can whisper things to Homura from time to time, and FF has Homura reserved. So she's just there to fill in that niche. Barring a nerf of some sort, her powers aren't anything to worry about.
I'm even having FF approve this instead of myself due to her powers.
