Physical Description (Living Form)
Jack is a White-Faced Capuchin who can generally be seen wearing some cutesy pirate inspired garb tailored to fit his little body. He's got the nasty fangs that could take a finger off that is usual for his species.
Physical Description (Undead Form)
Jack appears as a skeletal zombie with bits of flesh hanging off him, although his eyes are in perfect condition. His outfit is in tatters and barely hanging on his body.
Weapons
Jack is a monkey. Jack does not carry weapons on him, although he'll sometimes pick them up and appears to have the ability to use them.
Abilities/Powers
Jack carries the curse of Cortez' treasure, and as such is functionally immortal. In theory he could be killed if you smashed him completely, but no one's ever really tried that. Taking him apart won't kill him, his limbs will act independant of each other and his head will attempt to get his body parts back together. His true form is actually the skeletal form, but this can only be seen in moonlight.
Related to the curse, Jack can't feel any physical sensations, nor can he drink or eat anything. For some reason this doesn't stop him from trying, but he gets no flavor or nutrition from anything he consumes.
Skills
Jack is a very smart monkey. Barbossa has trained him to do all sorts of tricks and whatnot, and he's shown himself clever enough to help light cannons and shoot guns, although his aim is predictably abyssmal. He's also very good at following directions if he chooses to do so.
Personality
Jack is just a little pile of mischief who tends to get very upset when he's told he can't have things. He's a vicious little monster if he doesn't like you and seems quite content to lead people straight into the jaws of death if they get on his bad side. He really doesn't like Jack Sparrow (given the man has shot him multiple times, this is fairly understandable), but he loves Barbossa to pieces and if he can understand what Barbossa wants of him, he'll generally do it.
History
Barbossa picked up Jack at some point after committing mutiny aboard the Black Pearl against her captain, Jack Sparrow. He was with Barbossa when they found the treasure of Cortez and became cursed along with the rest of the priate crew. He served as a nuisance and a way to tease Jack Sparrow, and was freed from the curse along with the rest of the mutinous crew of the Black Pearl... but stole part of the treasure after this and became cursed again after Barbossa was killed by Jack Sparrow.
Jack fled to the Black Pearl and was nothing but a walking pile of stress relief for Jack Sparrow, who discovered the monkey's regained immortality when he shot him out of frustation one night. After extreme amounts of effort Jack Sparrow was able to capture Jack and used him as a bargaining chip with the sealed form of the sea goddess Calypso, Tia Dalma. He escaped shortly after this and reunited with Barbossa after Tia Dalma revealed that he was alive again (although she didn't explain it was due to her own magics).
When Barbossa once again stole the Black Pearl from Jack Sparrow, Jack was with him. Somehow or another though, Jack would end up on the Island....
